The poster is a practitioner quick-reference resource produced by the Child Safeguarding Practice Review Panel as part of its July 2026 adolescent suicide learning materials. It summarises key safeguarding messages from reviews of serious incidents involving adolescents who died by suicide, emphasising that suicide risk usually develops over time through cumulative harm, adversity and escalating vulnerability, rather than being caused by a single event. The poster encourages professionals to look beyond the immediate concern, use chronologies and multi-agency information sharing, keep the child’s voice central, remain professionally curious, and recognise that distress may present differently in neurodivergent or disabled children. It also highlights groups who may need particular attention, including children experiencing bullying, discrimination, social isolation, SEND, family adversity, poor school attendance, or involvement with children’s social care. The key practice message is that self-harm and suicidal thoughts should be treated as safeguarding concerns, with agencies maintaining consistent support, sharing information effectively, and understanding risk within the context of the child’s lived experiences over time.
